5.对个人研究领域的描述

手足口病(hand, foot and mouth disease, HFMD)是一种由肠道病毒感染引起的常见儿童传染病。该病传染性强、流行范围广、易出现暴发,重症病例病死率高,近年该病流行呈上升趋势。能够引起HFMD的肠道病毒有20多种(型),其中肠道病毒71型(Enterovirus 71,EV71)是引起重症HFMD的主要病原体。研究表明,年龄、家庭护理、发热持续时间≥3天、体温≥37.5℃、嗜睡、呕吐、高血糖、中性粒细胞数增加能够增加重症HFMD发病风险。关于重症HFMD的发病机制,国内外研究显示,中枢神经系统感染是重症HFMD的突出特征,且多种免疫细胞和细胞因子参与了重症HFMD的发病进程。目前关于HFMD研究存在的问题是:HFMD的病原谱、感染谱等流行特征、发展规律及主要影响因素不清晰,导致该病预防控制策略和措施的针对性、精准性和有效性大大下降,发病率常年居高不下;HFMD重症化机制不明,也无早期诊断方法,使重症化的早期干预难以施行,不能有效降低该病的病死(残)率。本实验室主要致力于研究HFMD流行特征及影响因素、HFMD病原谱与EV71变异规律、EV71侵袭中枢神经系统及诱发“瀑布样”炎症反应机制、肠道病毒感染诱发天然免疫逃逸机制、肠道病毒感染动物模型构建及疫苗研发。
