5.对个人研究领域的描述

肥胖是众多慢性非传染性疾病(如糖尿病,高血压,脑卒中等)的危险因素。肥胖人群因其体内脂肪过度蓄积常伴有胰岛素抵抗、血脂异常等代谢异常特征,被称为代谢异常肥胖。在全球20岁及以上成人中,约35%的肥胖人群表现为良好的代谢状态,即“代谢健康肥胖(Metabolically healthy obesity,MHO)”表型。MHO表型与人群健康的关系仍不完全清楚,且与心血管疾病发病风险的研究结论尚不一致。目前,基于西方人群有关MHO患病情况及其与心血管疾病和糖尿病发病关系的研究报道较多,缺乏有关中国人群的研究。同时,既往研究的结论并不完全一致,且存在代谢健康的诊断标准不统一及忽略腹型肥胖类型等问题。此外,人体的肥胖和代谢状态会随着年龄的增长及生活方式的改变而发生变化,有关肥胖和代谢状态的动态变化转归及其与心血管疾病发病风险的研究证据十分有限。2014年,中国成人肥胖人数居世界首位。中国农村地区肥胖和心血管疾病患病率呈上升趋势。我国农村人群MHO表型患病情况如何?MHO是否增加心血管疾病发病的风险?不同代谢特征人群长期的肥胖和代谢特征动态变化水平是否影响心血管疾病的发病?基因以及外环境危险因素如何影响MHO与心血管疾病发生的关系?这些科学问题至今未被研究。基于以上研究背景,本课题组采用前瞻性队列研究设计,选择河南农村地区成年人群为研究对象,通过建立基线研究以及开展多次长期随访调查,旨在从代谢的角度深入探索肥胖和人群健康的潜在关联。本课题的研究结果将有助于科学制定基于河南农村人群的心血管疾病预防控制策略,缓解心血管疾病的防治压力,同时为今后针对我国农村人群心血管疾病防治干预项目的开展提供前瞻性的科学依据。
